Asset Verification Form

The asset verification form should be used to update the status of any technology asset/device that was provided to you by the institution. The status of the device includes its location, date last seen, and condition. To report missing device please use this form located here:

To complete the Asset verification form:

1. Go to: Service - Asset Management
2. Once on the Asset Verification Service page, in the upper right select:

3. Select the date that the device was last seen:

4. Input your information into the fields below:

 

5. Use the comments section to note any damage to the device, provide the serial number if an asset tag is missing, or share other relevant concerns. You can also use this section to report assets whose ownership is unknown.

 

6. Select Submit
